FYI: You can only request this clearance for free once every 57 months. If you need another copy of it before your 57 months, it will cost $13. You should be able to log into the Keystone website and view your old clearances if they are not expired.
This form is available to print and sign. These are also available in the blue binder in the office. Once signed, please leave in the binder pocket or in the PTO mailbox for the secretary team to log. Clearances are valid for 5 years from the date of request.
If you have lived in PA for the past 10 consecutive years, you DO NOT need this clearance. If you do, this background clearance costs $22.60. FBI clearance code is 1kg6xn
